Terms and Conditions
1 A minimum of 24 hours notice is required when an appointment is cancelled. If this is not given a £20 charge may be applied to your account.
2 Patients covered by private medical insurance must please ensure, as well as a policy/membership number we are given your authorisation code when you commence treatment.
3 Would patients covered by private medical insurance please give reception details of a credit or debit card (to be held securely on your records). This information is required in the event that the private medical insurance company decline to pay an invoice. This information is strictly confidential, securely held by LB Physio and will not be disclosed to a third party.
4 Self-funding patients are expected to settle their account after each session.
5 If your personal details change please inform reception as soon as possible so your records can be updated.
6 You will be asked to complete a medical questionnaire on your first visit. Please inform the physiotherapist as soon as possible of any changes to your health or list of prescribed medication.
7 Please ensure you are appropriately dressed for physio as you may be asked to undress to your underwear.
